If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. We accept most major credit cards, including Visa, MasterCard, Discover, American express and CareCredit.
We are pleased to offer CareCredit dental financing to our patients. CareCredit is a convenient, no initial payment, low monthly payment plan for dental treatments of $500 to $40,000. CareCredit is a third party financing may be available for patients requiring extensive treatment. This type of financing must be approved in advance. The terms of this contract consist of six equal installments, free of interest or finance charges. The total financed amount however, must be paid in full within 6 months.
Our goal is to make our services affordable and accessible to our patients, therefore we accept most PPO and indemnity plans. We are Preferred Providers for Delta Dental, MetLife, Aetna, Cigna, United Healthcare, United Concordia, GEHA and many other Dental PPO plans. If you have dental insurance, we file claims as a courtesy for our patients.
